Ignite High Registration Form

Parental Consent (for delegates under the age of 18): I give permission for my child to attend Ignite Conference in 2024.

Participation: I agree that my child will cooperate with any instructions and behavioural expectations provided in regard to the Emmanuel Community Event (Ignite Conference 2024) that he/she is attending, and that I am responsible for any activities my child may choose to participate in during the Event. I understand that if my child is unable to remain at the Event due to medical, behavioural, disciplinary or other reasons, I will be responsible for any costs or other arrangements required for my child’s departure.

Indemnity: I understand that while every reasonable precaution will be undertaken to ensure the protection of my child, I hereby release Emmanuel Community and Stella Maris Catholic Parish, its Ministries, authorised Staff and Volunteers (“Emmanuel Community”) from any and all liability in the event of any injury, accident, misfortune, damage or loss that may occur to my child and/or their property while present at the Ignite Conference. Further, I indemnify Emmanuel Community and Stella Maris Catholic Parish from and against all loss, including legal expenses, connected with or arising from any claims or demands in relation to my child’s attendance at Ignite Conference, including leaving the conference venue without permission. I agree and understand that Emmanuel Community reserves the right to exercise discretion to refuse to register any delegate upon medical or other grounds without providing a detailed reason for doing so.

Medical Treatment Consent: I give permission for Emmanuel Community and Stella Maris Parish Catholic Parish to obtain emergency medical, hospital or ambulance assistance and/or treatment for my child at any time they consider necessary. I understand that every effort will be made for myself to be notified before instituting such procedures. I acknowledge that I will be liable for any medical, hospital or ambulance expense incurred in my child’s treatment, and I agree to pay those expenses. I have informed Emmanuel Community and Stella Maris Catholic Parish of any allergies or other medical conditions of my child that are relevant to this activity and will make any necessary medication available. Unless I have otherwise advised Emmanuel Community and Stella Maris Catholic Parish in writing, I give permission for Paracetamol to be administered to my child, at the discretion of the Ignite Conference First Aid officers and Stella Maris Parish Youth Leaders.

Privacy Declaration: I understand that Emmanuel Community and Stella Maris Catholic Parish may collect information about me/my child in accordance with their Privacy Policy, and that this information may be disclosed to certain parties it engages to organise the event. I consent to these details being used by Emmanuel Community and Stella Maris Catholic Parish, and other host organisations where applicable, for the promotion of future events and resources via post, phone, email and SMS. I can elect to have my details removed from their database at any time.

Media Release: I understand that Ignite Conference will be captured in photographs, video and audio, and that Emmanuel Community and Stella Maris Catholic Parish reserves the right to use this material for promotional purposes or other resources. I understand I must advise Emmanuel Community and Stella Maris Catholic Parish in writing if I do not give permission for images of my child to be used. I understand I may be visible in photos or video footage of crowds or large groups.
